Housing Stabilization Services is a Medicaid benefit available to people with disabilities and seniors in Minnesota find and keep housing. The Service is designed to address barriers to stable housing and increase integration opportunities. Our experienced Housing Specialist can assist and support individuals find and sustain affordable housing. People who need HSS services can self-refer to Radiant Care or be supported by a provider or case manager.
Services include:
Housing Transition
We can provide support with all the details associated with finding and moving including but not limited to:
- Finding suitable housing that fits the client’s profile
- Developing a plan and budget
- Assistance with the application process and leasing
- Coordinating moving
Housing Sustaining
This service also allows us to follow up and support individuals after moving into stable housing. Support may include but not limited to:
- Education and Coaching to develop and maintain key relationships with property managers and neighbors
- Educating and training on roles rights and responsibilities of the tenant and property manager
- Supporting the person to apply for housing benefits and resources
- Arranging for adaptive house-related accommodations
